St. Joseph's Medical Center Foundation Community Health Grant Application
In accordance with our mission and vision, St. Josephs Foundation acts as a catalyst in identifying and addressing the root needs of the communities we serve in a collaborative fashion with St. Josephs Medical Center, other health care providers and public agencies.
The Benedictine values and charism are expressed through our commitment to promote the health of the residents of the area with special concern for the poor and powerless.

Eligibility and Focus
Community Health Grant proposals must focus on community needs within the St. Joseph's Medical Center service area, be a non-profit or governmental agency and meet one or more of the following criteria:
- Provide health care services to patients and families who use St. Joseph's Medical Center and its clinics.
- Clearly show evidence of collaboration with organizations with similar missions for optimum community impact and value.
- Initiate/support health related programs in response to expressed community needs.
- Fund research and investigation into new and better ways of providing care, preventing and treating disease and promoting health.
- Fund health education for patients, families, professionals and the broader community.
Proposals must be consistent with the mission and values of St. Joseph's Medical Center and Foundation. The Foundation favors proposals that meet multiple criteria and that demonstrate a clear, direct, and measurable response to the health needs of our community. The Foundation reserves the right to alter its eligibility criteria and focus to meet compelling community health needs.
Ineligibility
The Foundation will not make contributions for:
- Incomplete proposals
- Political organizations or campaigns, or for organizations designed primarily for lobbying and advocacy
- Individuals and their projects
- Religious organizations for direct religious activities
- Endowment campaigns
- Fraternal organizations, societies or orders
- Purchase of program advertising space, tickets or raffles
- Telephone solicitations
- Benefits or fundraisers; testimonial recognition or honoring events
St. Joseph's Foundation is reluctant to provide for ongoing operational support or general administrative expenses for existing programs. Similarly, organizations that have received funds from the Foundation will generally not be considered for future funding for 3 years after the initial award. The Foundation is reluctant to award funds for capital improvements (bricks & mortar) unless Foundation funds are specifically designated for this purpose.
Application Procedure
Proposals are reviewed quarterly, so please submit your proposal as early as possible.
Because our resources are limited, we cannot fund every request.
Adhering to our procedures and providing us complete information on your project will help us review your request.
To submit your request, please complete:
A Foundation grant does not guarantee further support nor is it a pledge for future support.
All successful grant applicants must submit a performance report to the Foundation within 30 days after project completion.
No further support will be considered unless a performance report is on file with the Foundation. By accepting Foundation funds, grant applicants agree to acknowledge the award in all appropriate printed materials, such as press releases, feature articles, donor recognitions, or by other suitable means.
Additional Guidelines
- Deadlines: Applications must be received by the first day of January, April, July and October.
- A Foundation representative may contact you for more information or to have an on-site visit to help clarify your proposal.
- The Foundation will notify all grant applicants of their status within 30 days after the Grant Review Committee meets.
- In the event that a grant proposal is funded and the applicant is notified but makes no request for the funds or a deferral of the project in the succeeding twelve months, the applicant must submit a new grant proposal for consideration.
